Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:      (a) On or about September 13, 2016, for the employee who sustained fatal injuries after connecting a steel column to a Sky Trak, model 3606, serial number 11847, rough terrain forklift; the employee was run over as the forklift moved forward at 1800 East 57th Street North, Sioux Falls, South Dakota.                                           Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be for the employer to apply the manufacturer's operator and safety manual for Preparation, Prevention and Clearances and ANSI/ITSDF B56.6, Section 6.2, paragraph 6.2.6 which states: Understand rough terrain forklift truck limitations and operate the truck in a safe manner so as not to cause injury to personnel.
