On September 14, 2016, an employee was inspecting an electrical cabinet, where there were exposed electrical busses, without first assuring the electrical cabinet was de-energized. 29 CFR 1910.333(b)(2)(iv): Qualified persons did not use test equipment to test the circuit elements and electrical parts of equipment to which employees were exposed and verify the circuit elements and equipment parts were de-energized.    On or about September 14, 2016, employees were troubleshooting an electrical system which had experienced a power outage without first verifying electrical power source was de-energized.  Employees were exposed to electrical shock hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards did not use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.     On or about September 14, 2016, employees were not provided with gloves to protect the hands in the event contact was made with live electrical parts.  Employees were exposed to electrical shock hazards.
