29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) Holiday Inn - On or about September 14, 2016, employees were not adequately trained to wear their personal fall arrest systems. Employees did not know how to properly adjust the harness straps and dorsal D ring. Employees were wearing personal fall arrest systems while using a JLG aerial lift to pressure wash the hotel.
