29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about September 15, 2016 and at times prior thereto, employees who were performing roofing construction with hand tools and pneumatic air guns were not wearing eye protection. This condition exposed employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about September 15, 2016 and at times prior thereto, employees performing new residential roofing construction were exposed to fall distances of greater than six feet without fall protection on a steep roof. This condition exposed employees to the hazard of falling to the ground below.
