29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about September 13, 2016, the rotating drum and other rotating parts on carding machines were not effectively guarded in that the interlocked barrier guard allowed for exposure to the rotating parts when opened.

29 CFR 1910.219(m)(1)(i): Standard conditions shall be secured by the use of the following materials. Expanded metal, perforated or solid sheet metal, wire mesh on a frame of angle iron, or iron pipe securely fastened to floor or to frame of machine. On or about September 13, 2016, pulleys on carding machines that were less than seven feet from the floor were not effectively guarded in that the interlocked barrier guard allowed for exposure to the rotating parts when opened.
