29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about September 15, 2016, the employer failed to develop, document, and utilize machine specific procedures for equipment within the facility including but not limited to, #15 Slitter, #19 Staley FFG, #20 McKinley RDC, and the #700 Martin FFG which employees were required to perform LO/TO for the control of hazardous energies. Hazardous energy sources include but are not limited to electrical, mechanical, hydraulic, and pneumatic.    NOTE: Employer must comply with 29 CFR 1910.147(c)(6)(i)(C), when procedures are developed.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    On or about September 15, 2016, the employer failed to ensure that employees were trained on the control of hazardous energies. Employees tasked with the LO/TO of equipment for maintaining and servicing had not received authorized training. Employees tasked with operating and working near equipment had not received affected training. Energy sources included, but are not limited to, electrical, pneumatic, hydraulic, etc. Employees are tasked with maintaining, servicing, and operating equipment including but not limited to #15 Slitter, #19 Staley FFG, #20 McKinley RDC, and the #700 Martin FFG.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):    On or about September 15, 2016, the employer failed to ensure that the electrical,  pneumatic, hydraulic, and mechanical energy disconnects for the #15 Slitter, #19 Staley FFG, #20 McKinley RDC, and the #700 Martin FFG were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each specific energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safer during servicing/ maintenance and set up operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about September 15, 2016, the employer failed to ensure that machine guarding was present on the #15 S & S Slitter. There were no barrier guards around the moving metal rollers of the slitter.    b) On or about September 15, 2016, the employer failed to ensure that machine guarding was present on the #19 Staley FFG . The back side of the stacker was not guarded to prevent employees from accessing the moving belts.    c) On or about September 15, 2016, the employer failed to ensure that machine guarding was adequate on the #20 McKinley RDC.  Areas of the line were not guarded effectively. Barrier guards were easily removable, there were no interlock devices and areas which contained machine hazards were not effectively guarded. The line includes sections such as but not limited to lay boy area (orange belts) and the stacker unit.    d) On or about September 15, 2016, the employer failed to ensure that machine guarding was present on the #700 Martin FFG . The accumulator center section was not guarded to prevent employees from accessing the moving/rolling parts.
