29 CFR 1910.178(q)(1):     Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a)  Work place was located at 125 County Road, Jersey City, NJ 07307     H&M International Transportation employees were exposed to injuries as they were operating a side loader with known maintenance issues that should have required the machine to be removed from service until repaired, including but not limited to the twist lock green indicator light.                                          Violation occurred on or about 8/15/2016.
