29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:( a) BAE USS McFaul: On or about 09/12/2016, an employee wore a MSA NIOSH-certified Abrasi-Blast respirator that was not used in compliance with the conditions of its certification in that a MSA constant flow valve was not attached to the breathing tube. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel. a) BAE Systems, USS McFaul: On or about 09/12/2016, employees plugged a light into extension cords supplied by a 110 volts breaker panel that was not provided with ground fault interrupter protection therefore employees were exposed to possible burns resulting from electrical shock. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
