On or about September 12, 2016, on a 5:12 pitch roof, employees performing roofing operations (tearing off and installing shingles), were not protected from an 18 1/2 foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection. b. On or about September 12, 2016, on a 5:12 pitch roof, an employee performing roofing operations (installing shingles), was not protected from an 8 1/2 foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
