Crossbraces were used as a means of access.   (a) Second Floor - On or about September 14, 2015 an employee gained access to the homemade wooden scaffolding by climbing up the side wood brace. Scaffolding walk plank is 6.5 feet from floor open rafters that is 10 feet from ground.  Employee was exposed to falling 16 feet to a lower level.  Employee was not provided a ladder or other safe point of access.
