29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a. Falcon Condominiums, 3863 Terrace Street, Philadelphia, PA 19128 - The employer did not ensure that a subcontractor employee, who was performing masonry work from a ladder jack scaffold, used fall protection. The employee was working approximately twelve (12) feet above the ground and was exposed to a fall hazard. Observed On or About 08/09/16
