29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Piper Electric, Co. Inc. at 2442 West 121st, Westminster, CO, 80234: On or before September 13th, 2016, the employer did not ensure that an employee working on an elevated platform was protected from falling through the use of a guardrail system, safety net system, or a personal fall arrest system. An employee was working on the catwalk in the attic of the Westminster First Citizens Bank, installing and pulling fiber optic and computer cable. The catwalk did not have guardrail and toeboard protection on an open end. The employee fell from the catwalk and through the ceiling, approximately 15 feet, and incurred lower body injuries. This condition exposed employees to a fall hazard of 15 feet.
