29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Back Side of House - On or about September 14, 2016 employees removing old asphalt shingles from steep roof over 9 feet high were not protected from falling using a guardrail system or safety net system or personal fall arrest system.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. (a) Roof, Back Side of House - On or about September 14, 2016 employees removing old roofing shingles were holding on to a rope that was fastened to the ridge using nails bent over onto the rope. All ropes were not fastened to the ridge. They were placed over the ridge to look like they were attached.
