29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) East side of roof - On or about September 14, 2016 employees applying plywood decking on roof rafters 9 feet high were not protected from falls by using any fall protection.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. (a) East side of house - On or about September 14, 2016 employees applying 4 x 8 sheets of plywood to roof were not wearing any safety glasses to protect them from eye injuries using air nail gun at 100 psi and wearing hard hat to protected them from head injuries lifting plywood up to roofers on roof 9 feet high.
