Employees were exposed the hazard of a 15 foot fall.  b) On or about 09/19/16 at 375 Melville Street, Rochester, New York; employer did not inspect the job site to ensure employees working on a ladder jack scaffold were wearing a safety harness and tied off to a suitable anchorage point. Employees climbed the power ladder (platform material hoist) to access the ladder jack pic scaffold on the west side of the house to remove the roofing around the eye brow area exposing them to a fall hazard of approximately 15 feet and without any rules posted informing employees not to climb the power ladder. 29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less:  a) On or about 09/19/16 at 375 Melville Street, Rochester, NY; the scaffold system consisting of a Werner brand ladder jack scaffold, Model WNS-2424, with a rated load of 500 pounds and two support ladders ( a Louisville and a Werner brand 24 foot each with a 300 pound capacity ) was overloaded when three workers, cumulatively weighing approximately 695 pounds were directed onto the scaffold at the same time resulting in a collapse of one of the support ladders with all employees falling approximately 15 feet.     ABATEMENT DOCUMENTATION REQUIRED
