29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years: On 09/19/2016, four employees working at Proffer Produce located at 920 5th Street, Park Hills, MO 63601, were operating forklifts without having completed the required refresher training. The operators had last attended training with dates of 12/12/2012, 12/13/2012 and 03/31/2013, but had not completed the 3 year refresher training/evaluation since.
