On or about September, 19th 2016, at the above addressed job site, employees were not protected from an approximate 10 feet fall while installing plywood.

On or about September 19th, 2016, at the above addressed job site, employees were exposed to a 10 feet fall hazard when using an extension ladder which did not extend at least 3 feet above the upper landing surface while gaining access to a second floor.
