29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):    The employer is failing to protect employees from an engulfment hazard associated with entering a trench without adequate protective systems.  The most recent occurrence of this was observed at 15010 S State Hwy 31, Gretna, NE where an employee was observed working in a 9 foot deep trench that was not benched or sloped, nor did it have protective systems in place to protect against sloughing or cave in hazards.

29 CFR 1926.651(i)(1): Where the stability of adjoining buildings, walls, or other structures was endangered by excavation operations, support systems such as shoring, bracing or underpinning was not provided to ensure the stability of such structures for the protection of employees:    The employer is failing to protect employees from struck by hazards by not providing a suitable support system to a power pole adjacent to an excavation operation.  The most recent occurrence of this was observed at 15010 S State Hwy 31, Gretna, NE 68028 where the employer dug a trench next to a power pole, potentially compromising its stability by removing 9 feet of soil at its base.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    The employer is failing to provide employees with a safe means of egress out of a trench excavation.  The most recent occurrence of this was observed at 15010 S State Hwy 31, Gretna, NE 68028 where an employee was in a trench that was over 9 feet deep.  The employee had climbed into the trench and there was no ladder, stairway, ramp, or other safe means of egress.

The most recent occurrence of this was observed at 15010 S State Hwy 31, Gretna, NE 68028 employees where exposed to chlorine and diesel fuel and the employer had not established a HAZCOM program or provided SDSs.
