29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about September 13, 2016, Employees were exposed to fall hazards on a construction site in Jacksonville, Illinois, and the employer did not ensure aerial lift operators were trained, as necessary, by a competent person.      The training should include, but not limited to:     a)      Explanations of overhead electrical power lines, fall, and falling object hazards;   b)      Procedures for dealing with hazards;   c)      Recognizing and avoiding unsafe conditions in the work setting;   d)      Instructions for correct operation of the lift (including maximum intended load and load capacity);  e)      Demonstrations of the skills and knowledge needed to operate an aerial lift before operating it on the job;  f)       When and how to perform inspections; and g)      Manufacturers requirements.

The employee was wearing a harness and lanyard, but was not attached to the aerial lift.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):    On or about September 13, 2016, Employees were exposed to fall hazards on a construction site in Jacksonville, Illinois, and the employer did not ensure fall protection training was conducted by a competent person qualified in the following areas:     a)      The nature of fall hazards in the work area; b)      The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;  c)      The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; d)      The role of each employee in the safety monitoring system when this system is used; e)       The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and f)      The standards contained in this subpart.
