29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a.) Scott Systems International, LLC dba RobotWorx located at 370 W. Fairground St. Marion, OH; On or about September 19, 2016, the employer did not ensure employees were trained in and familiar with electrical safety related work practices before changing 480V fuses for the Bay 5 Slot 1 transformers. Employees were required to pull fuses before performing maintenance on the 480V panel for the transformers and did not have any training on Electrical Safety Related Work Practices.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a.) Scott Systems International, LLC dba RobotWorx located at 370 W. Fairground St. Marion, OH; On or about September 19, 2016, the employer did not ensure employees utilized electrical safety related work practices while pulling 480V fuses for the Bay 5 Slot 1 transformers. Employees were required to change fuses on the 480V panel for the transformer and were exposed to electric shock and arc flash hazards.

29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: a.) Scott Systems International, LLC dba RobotWorx located at 370 W. Fairground St. Marion, OH; On or about September 19, 2016, the employer did not ensure employees were protected while pulling six 480V fuses for the Bay 5 Slot 1 transformers. Employees were required to pull the fuses after a fuse was blown, and did not verify zero energy with a meter, before performing work and did not wear the required face, eye, hand, and body protection, exposing them to electrical shock and arc flash hazards when pulling the fuses with fuse pullers.
