29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a)On or about 19 September, 2016, an employee was working on the form work of the building under construction, while sitting on top of the cement blocks on the third floor without fall protection, exposing the employee to a fall hazard of approximately 29 feet.   (b) On or about 19 September, 2016, four employees were working on the top floor of the building under construction and were walking near an open area without any guardrails, exposing the employees to a fall hazard of approximately 9 feet unto the floor beneath.

29 CFR 1926.501(b)(4): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about 19 September, 2016, at the above addressed jobsite, employees were walking/working around open holes, without the use of fall protection, exposing the employees to a fall hazard of approximately 9 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about 19 September, 2016, at the above addressed jobsite, employees were accessing the top floor of the building under construction by utilizing a "job-made" ladder that did not extend at least 3 feet above the upper landing surface, exposing the employees to a fall hazard of approximately 9 feet.
