29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Main shop area. on or about August 30 2016 and at times prior, the employer exposed employees to a caught in hazard in that the Edward Segal Grommet Machine used a foot operated control switch instead of a two hand control that allowed employees hands into the point of operation.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) Main shop area. on or about September 20, 2016 and at times prior the employer exposed employees to a heat hazard in that employees were operating a Eastman drill marker that had exposed wires coming into the top.
