29 CFR 1910.23(a)(8)(ii): Every floor hole into which person can accidently walk was not guarded by a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing:     Jobsite:  The floor hole, in between the loading dock and the end of truck bed was not protected by a cover, such as (but not limited to) a loading dock plate or an equivalent.    On or about July 22, 2016, an employee, who was manually unloading sliding glass from the truck bed onto the loading dock tripped into the gap/opening and fell to the ground. The employee sustained an injury, twisting the back.
