29 CFR 1910.178(m)(10): A load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward. Bean receiving area - On or about September 20, 2016, employees were allowed to operate the Raymond sit down forklift (serial No. 455-14-12402) without a load backrest extension. Employees were thereby exposed to struck-by hazards from falling materials.

29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted. Production areas - On or about September 20, 2016, employees were required to work in the following areas on a daily basis. Three emergency exit signs were not illuminated as required, thereby exposing employees to the hazards associated with fire and smoke. 1. Main building 4th floor southeast 2. Basement northeast 3. Bean receiving room
