29 CFR 1910.219(f)(1)(i): Gears were not guarded by a complete enclosure.     a. Production Area- On or about September 20, 2016, in the facility employees were required to pick up and sort packaged meat products rolling down a moving conveyor belt. The employer did not ensure that all the rotating gears were guarded by a complete enclosure.  The employees were thereby exposed to cuts, lacerations, and amputation hazards.

On or about September 20, 2016, on the 2016 OSHA 300 / 300A log the employer did not enter the job title for each recorded injury, the annual average number of employees covered and the total hours worked by all employees.
