29 CFR 1910.147(a)(2)(ii)(B): An employee was required to place parts of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. a) Sweetwater Building, Rewinder Macihine- On or about September 14, 2016, and at times prior, the employer exposed employees to caught-in hazards while threading a splice of paper in the Beloit 414-G-84 Rewinder machine. The employer and employees indicated that the operation was classified as a normal routine operation but did not establish and/or implement an alternative procedure or measure to protect their workers.
