29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from the hazards associated with broken bones and potential amputations. One instance, which occurred on or about September 15, 2016, involved an injection molding machine in the facility located at 6110 Abbott Dr., Omaha, Nebraska. During a routine clearing operation, employees are exposed to these potential hazards from the injection molding machines. During operation the auger feeding plastic pellets into the heating element portion of the injection molding machine becomes jammed with excess melted plastic which hasnt been pulled through the system. Employees are required to clear this jam. To do this they need to open the system while the auger is running allowing employees to place their hands in the area of the machine which creates an amputation/crushing hazard.
