29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, items A-D of this section: a) NAS Lemoore, NAVFAC Public Works Department (PWD): On or about June 15, 2016, the NAVFAC PWD Lockout/Tagout (LOTO) procedure failed to specify procedural steps for the placement for the placement, removal, and transfer of LOTO devices and the responsibility for them while work was conducted on Padmounted Switchgears. An employee suffered second and third degree burns after receiving an electric shock from the 12kv electrical system while attempting to replace the Padmounted Switchgear.

The latest inspection was completed April 15, 2015. An employee was exposed to an electric shock from 12kv electrical system which resulted in second and third degree burns.

An employee was exposed to an electrical shock from 12kv electrical system which resulted in second and third degree burns.

An employee was exposed to an electric shock from 12kv electrical system which resulted in second and third degree burns.
