29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) CK Manufacturing, LLC, 330 Millwood Road, Lancaster, Pennsylvania - Employees did servicing and maintenance activities on machinery and equipment, including, but not limited to, the 100-ton Niagara SC2-100-60-30 part-revolution mechanical power press, and the employer did not develop and implement an energy control program, consisting of machine specific energy control procedures, group lockout/tagout, periodic inspections, employee training, etc., exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, on or about September 9, 2016.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) CK Manufacturing, LLC, 330 Millwood Road, Lancaster, Pennsylvania - Employees did servicing and maintenance activities on machinery and equipment, including, but not limited to, the 100-ton Niagara SC2-100-60-30 part-revolution mechanical power press.  The employer did not develop and implement machine specific energy control procedures for the machinery and equipment, and employees did not lockout/tagout the machinery and equipment, for example, during die changes, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, on or about September 9, 2016.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses:    (a) Shop - Employees used a foot pedal to actuate the 100-ton Niagara SC2-100-60-30 part-revolution mechanical power press, and this press had no point of operation guarding and no protection by point of operation devices, exposing employees to getting caught-in/struck-by the point of operation, as happened on or about September 9, 2016 when an employee suffered amputation injuries to the left middle and index fingers.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    (a) CK Manufacturing, LLC, 330 Millwood Road, Lancaster, Pennsylvania - An employee suffered amputation to fingers of the left hand, on September 9, 2016 as a result of getting the hand caught-in/struck-by the point of operation of a mechanical power press.  The employer failed to report this work-related hospitalization to OSHA within 24 hours.  (The employer reported this hospitalization to OSHA on September 15, 2016.)    Corrected during the inspection.
