29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board:    a) On or about September 23, 2016, employees were exposed to the hazard of falling from a pitched roof while on a crawling board (chicken ladder) located on the roof of a two story building.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):        a) On or about September 22, 2016, employees were exposed to the hazard of falling while working from the roof of a two story residential structure without fall protection.        Esteban Garcia dba Garcia Roofing Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1018656, citation 1, item 2b, and was affirmed as a final order on June 4, 2016, with respect to a worksite located at 2300 Ash Lane, Northbrook, Illinois.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
