29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Rodriguez Roofing Services Inc., 584 County Rd 697, Grand Lake, CO 80447: On and before September 16th, 2016, employer did not ensure employees were provided with eye protection when using pneumatic nailers during roofing operations on a two story residential property. The condition exposed the employees to the hazard of projectiles to the eye.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or a personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Rodriguez Roofing Services Inc. at 584 County Rd 697, Grand Lake, CO 80447: On and before September 16th, 2016, the employer did not ensure that each employee on a roof with unprotected sides and edges 6 feet or more above lower levels was protected from falling. Employees were using pneumatic nail guns to shingle this roof with a 6 on 12 pitch without the use of fall protection. This condition exposed the employees to a fall hazard of up to approximately 30 feet.
