29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on September 22, 2016 on the jobsite located at 4221 Pinewood Lane, Lincoln, NE 68516. Employees were observed reroofing a residence on a 8:12 pitched roof at heights greater than 6 feet above the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The employer is failing to ensure that all employees are protected from fall hazards when entering/exiting the landing surface of a residential roof. This most recently occurred on September 22, 2016 on the jobsite located at 4221 Pinewood Lane, Lincoln, NE 68516. Employees were observed using ladders that did not have side rails which extended to at least 3 feet above the landing surface, which is approximately 12 feet above the ground, when entering/exiting the roof to perform reroofing duties.
