The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a. Site - Employees performing roofing work were not adequately protected from falls of approximately 20 feet when vertical lifelines that had been tied around employees waists were used as fall protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Site - Employees performing roofing work were not protected from falls of approximately 20 feet when fall protection was not used. Lc Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1926.501(b)(13) which was contained in OSHA inspection number952277, citation number 001, item number 003 and was affirmed as a final order on 5/1/2014, with respect to a workplace located at 70 Turtle Brook Rd. Canton, MA 02021.
