29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) (DESCRIBE HAZARD(S) WHERE NECESSARY)  On or about September 15, 2016:  a) Employees were exposed to chemical hazards associated with liquefied propane gases, when the employer failed to properly asses the workplace and provide the proper personal protective equipment to eliminate the hazard.  b) An employee was exposed to chemical burns when battery acid overflowed onto the floor of the battery charging area, when the employer failed to properly asses the workplace and provide the proper personal protective equipment to eliminate the hazard.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
