29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: On or about 17 May, 2016, at the above addressed jobsite, employees were working from a billboard sign and were tying off unto a support that was not capable of supporting at least 5,000 pounds per employee, exposing the employee to a fall hazard of approximately 20 feet.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about May, 17, 2016 at the above addressed jobsite two employees were working on a billboard without the use of a ladder to reach the top level of the billboard from the catwalk, exposing the employees to a fall hazard of approximately 20 feet.
