(b) Warehouse Area, on or about and at times preceding to September 23, 2016, Toyota model #7FGU35 serial #61148 was in disrepair in that hoses were leaking, engine oil leaked, gasket on hydraulic tank and radiator cover were defective, and the forklift was not removed from service until safe to operate. (c) Warehouse Area, on or about and at times preceding to September 23, 2016, Toyota model #7FGCU55, serial #6200 was in disrepair including headlights, tail lights, pedal pads, and the engine trouble code, and the forklift was not removed from service until safe to operate.
