29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Two employees were exposed to falls of approximately 18 feet to a lower level while conducted roofing operations on a 6 in 12 pitch roof. The two employees were not tied into an anchor point while wearing harnesses.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder: An employee was exposed to a fall of approximately 9 feet to a second roof level, while progressing up an extension ladder and carrying a large plastic bag of roof vents. The employee did not grasp the ladder during this climb and all times.
