29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: (a) On or about September 21, 2017, the clerks and their supervisors had operated power driven hand jacks (PHJ) to move gaylords and APCs. They had not received training to operate the PHJ, and there were some employees that were operating in an unsafe manner by hooking multiple APCs together and moving them with the PHJ.
