Employees were working on a 6:12 pitch roof without the use of a fall protection system. Employees were exposed to a fall height of approximately 9 feet 4 inches.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) For employees working at 4008 Sebring Parkway in Sebring FL as observed on or about 22 September, 2016. Employees were working on the roof of the residential home cutting plywood sheathing with a circular saw that was powered through an extension cord plugged into a permanent exterior outlet. The extension cord was missing the ground prong.
