29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    At a commercial construction site, employees were installing foam panels to the exterior walls of a commercial building under construction on incomplete platforms of tubular frame scaffolds.  The working platforms were not fully planked; exposing employees to fall hazards.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    At a commercial construction site, employees used a tubular frame scaffold without base plates.  Employees were exposed to fall hazards 10 feet above ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    At a commercial construction site, a tubular frame scaffold located throughout a commercial building under construction and used by employees, did not have a ladder or other safe means to gain access to the scaffold platforms.  Employees were exposed to falls 10 feet above ground.

At a commercial construction site, employees were exposed to electrical hazards while using a tubular frame scaffold to install foam panels to the exterior walls of a commercial building under construction,1 1/2 feet from an overhead electrical power line.
