29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).    Location:  Ladder jack scaffold at the rear of the building, middle section.    A section of the ladder jack scaffold work platform consisted of one wood plank that did not fully plank the jack.  An employee working on it was exposed to a fall hazard of approximately 12 feet.

Location:  Ladder jack scaffolding at rear of building.    Employees working on ladder jack scaffolding were exposed to a fall hazard of approximately 12 feet.  The employees were not protected against falling to the ground below.
