29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 26 September 2016, and at times prior thereto, maintenance employees performing corrective maintenance on the pushers, including unjamming, for the Mail Staging Towers (MST) were not utilizing Specific Energy Control Procedures for Tray Management System (TMS) Mail Staging Tower (MST) for energy isolation, exposing the employees to the hazard of being caught in the machine. a) Pneumatic Energy: Compressed air is used to provide energy for the pneumatic pushers or rams that push trays or tubs either into or out of the tower through container ports. Employees are not isolating the pneumatic energy at the air valve. b) Kinetic: Multiple trays or tubs of mail on shelves move vertically up or down within the tower on a carousel with potential kinetic energy. Employees are not isolating the kinetic energy at the carousel.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 26 September 2016, and at times prior thereto, the pneumatic energy source for the Mail Staging Towers (MST) was not physically locked out by employees with a device at the energy isolating valve, exposing maintenance employees clearing jams near the pushers to the hazard of being caught in the machine. The pneumatic energy isolating device for the tower is an Air Valve located approximately 20 feet above the floor.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. On or about 26 September 2016, and at times prior thereto, maintenance employees performing corrective maintenance on the Tray Management System (TMS) Mail Staging Tower (MST) did so with the use of a specific energy control procedure that did not clearly and specifically outline the steps and means to block or dissipate stored energy, exposing the maintenance employees to the hazard of being caught in the machine.
