29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section:   a) Extruder Area - On or about September 22, 2016, the employer did not ensure equipment-specific energy control procedures were utilized for the control of potentially hazardous energy when an employee was conducting servicing and maintenance by unjamming a strip of plastic during the operation of the RDN Manufacturing Co. Inc., Intellicut model #C420A, serial #42027. This machine had electrical and pneumatic energy.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:   a) Extruder Area - On or about September 22, 2016, the employer did not ensure that there was an equipment-specific energy control procedure for the task of unjamming material from the RDN Manufacturing Co. Inc., Intellicut model #C420A, serial #42027, that included the following items: a specific statement of the intended use of the procedure (servicing, maintenance, and/or un-jamming), procedural steps for shutting down, isolating and securing the machine to control hazardous energy (electrical and pneumatic), procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices, tagout devices and other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Extruder Area - On or about September 22, 2016, an employee was exposed to machine hazards associated with moving parts and struck-by hazards while unjamming a plastic strip on the RDN Manufacturing Co. Inc., Intellicut model #C420A, serial #42027.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) Extruder Area - On or about September 22, 2016, the employer did not ensure an employee that unjammed a plastic strip on the RDN Manufacturing Co. Inc., Intellicut model #C420A, serial #42027, was trained as an Authorized employee.  The employee performed this servicing and maintenance task without the knowledge and skills needed for the safe application, usage, and removal of the energy controls.  This machine had electrical and pneumatic energy.
