29 CFR 1910.265(c)(26)(ix): Every stacker and unstacker shall be inspected at frequent intervals and all defective parts shall be immediately repaired or replaced. Kersten Lumber Co. Inc. kiln building located at 295 Hwy 45, Birnamwood, WI: On and before 8/19/16, the stacker had not been inspected by the employer to ensure proper working condition and did not identify and correct conditions which led to a roller chain jumping sprocket teeth, which in turn caused employees to adjust the timing while the machine was running.
