29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: This violation was discovered on or about September 20, 2016, when the employer did not establish a program consisting of energy control procedures to include but not limited to employee training and periodic inspections. Employees were exposed to a caught-in and amputation hazards.

29 CFR 1910.219(f)(3): All sprocket wheels and chains were not enclosed when less than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling was not provided. This violation most recently occurred on or about September 20, 2016 and at times prior thereto, when employees who worked on the portable debarker in the production areas with no protection from rotating parts were exposed to a caught-in hazard.
