29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:  (a) On or about September 22, 2016 and at times prior thereto employees were exposed to fall hazards greater than 33 feet while inspecting the roofs of aircraft sunshades on the flightline, where no abrasion protection was used to prevent the nylon vertical life line from cutting in two on the roof edge in the event of a fall.  Abatement certification and documentation are required for this item.
