(a) South end of trench at exit - On or about October 12, 2016 employees working in the trench must use ladder to enter and exit the trench by walking around open hole in man hole box.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. (a) Division St. - On or about September 27, 2016 an employee was working inside excavated hole 20 feet deep where water inside the hole with no means of removing the water has caused the sides to collapse below the trench box side supports.

29 CFR 1926.652(g)(2): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of a shield system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the shield. (a) North East side of trench - On or about September 27, 2016 employees working inside 20 feet deep trench installing new drainage pipe when the trench side shield plates were more than 2 feet above the bottom of trench and the side soil has blown out causing the soil to fall into the trench.
