29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On the southwest side of the establishment, an employee tasked with installing front channels on the parapet wall was observed operating an aerial lift, moving from the aerial lift basket to the roof, and moving from the roof to the aerial lift basket without tying off. The aerial lift basket was at a height greater than sixteen feet above ground level.
