29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:    On or about September 28, 2016 at the job site where an employee was exposed to a fall hazard of more than 6 feet while performing roofing activities without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about September 28, 2016 on the roof of the Taqueria Arandas restaurant an employee was exposed to a fall hazard when the ladder used to gain access to the work area did not extend at least three feet above the surface of the roof.
