29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about September 28, 2016 and at times prior, an employee was exposed to possible serious injuries from flying metal particles and point of operation of a Clausing lathe machine, Model #6903. The lathe machine was not provided with a chip guard cover.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a. On or about September 28, 2016 and at times prior, employees were exposed to laceration injuries while using a band saw that was missing a guard cover on the exposed blade of a Craftsman band saw, Model #11324261. The band saw was not equipped with a guard cover.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. On or about September 28, 2016 and at times prior, employees were exposed to electric shock from several junction boxes that were used pendant boxes to power electrical equipment in the production area. The junction boxes were not mounted and secured on a fixed location according to their listing and labeling.
